• Digital inquiry or contact forms submitted through our website.

• Initial and ongoing clinical consultations and physical assessments.

Where necessary, we may also receive information from your GP, specialist, or funding body, but only with your explicit consent.


4. Why We Collect and Use Your Information

We use your information strictly to provide safe, effective, and professional healthcare. This includes:

• Assessing, diagnosing, and treating your physical conditions.

• Communicating with you regarding appointments, billing, and clinical care.

• Writing necessary reports back to your referring doctors or funding agencies (such as NDIS or Medicare).

• Fulfilling legal, regulatory, and insurance requirements.

Your health records are kept strictly confidential. We will not disclose your personal or health information to anyone unless:

• You have given us clear, written consent to share it (e.g., sending a report to your GP).

• We are legally required or authorized to do so by law (e.g., subpoena, or mandatory reporting).

• There is a serious, imminent threat to your life, health, or safety, or to public health.

We take data security seriously. Your digital records are stored securely using encrypted, password-protected cloud platforms designed for health practitioners. Physical documents, if any, are kept in secure, locked storage. We retain health records for the legally required periods (typically 7 years from the date of last entry for adults).


7. Accessing and Correcting Your Information
